Patient Information

You should bring the following items to your first appointment:

  • ​A government-issued ID

  • Your insurance card

  • A list of your current medications, including dosages and frequency of use

  • Any medical records or test results from previous visits to other healthcare providers

  • A list of questions or concerns you have about your health

  • If you have a referral, bring that as well.

​

It's also a good idea to wear comfortable clothing that allows the doctor to easily examine the affected area, if necessary.

All insurance companies cover wound care services, but they may require prior authorization. The cost you are responsible for, such as a co-payment, co-insurance, or deductible, will be determined by your insurance company.

 

Our wound care providers accept government-sponsored plans like Medicare and Medicaid, as well as many commercial plans like Blue Cross Blue Shield, United Healthcare, Aetna, Humana, among others.

 

However, exceptions may apply, so please check with our Wound Care Center or your insurance company for confirmation.

Do you offer patient education?

Yes.


We believe that knowledge is power, and that's why we offer a variety of patient education resources. From informational brochures to in-person patient education sessions, we are here to help you learn all you need to know about wound and ostomy care.

Whether you are living with a chronic wound, a wound resulting from an injury, or a wound associated with another medical condition, we can help. Our team of experienced wound care specialists can provide the latest treatments and technologies to promote healing and minimize the risk of infection.

If you have undergone ostomy surgery, you may have questions about how to manage your stoma and ostomy bag. Our ostomy care specialists can provide guidance on skin care, pouch selection, and other aspects of ostomy management.

During your first appointment at the medical clinic, you can expect the following:

  • Initial assessment: The doctor or medical staff will ask you about your medical history, symptoms, and any current concerns you have. This will help the doctor gain a better understanding of your overall health.

  • Physical examination: Depending on the reason for your appointment, the doctor may perform a physical examination, which may include taking your vital signs (blood pressure, heart rate, and temperature) and examining the affected area.

  • Diagnostic testing: If necessary, the doctor may order diagnostic tests, such as blood tests or imaging tests, to help diagnose any underlying conditions.

  • Treatment plan: Based on the assessment and diagnostic results, the doctor will develop a treatment plan that is tailored to your specific needs. This may include a combination of medication, lifestyle changes, and follow-up appointments.

  • Discussion of results and next steps: The doctor will discuss the results of the assessment, diagnostic tests, and treatment plan with you and answer any questions you may have. You will also discuss any follow-up appointments or referrals that may be necessary.
