Patient Information

You should bring the following items to your first appointment:

​​

  • ​A government-issued ID

  • Your insurance card

  • A list of your current medications, including dosages and frequency of use

  • A list of your current ostomy supplies (pouches, skin barrier wafer and accessories)

  • A complete pouch change including accessories

  • Any medical records or test results from previous visits to other healthcare providers that can not be accessed through EPIC

  • A list of questions or concerns you have about your health

  • If you have a referral, bring that as well.

It's also a good idea to wear comfortable clothing that allows you to be examined easily.

All insurance companies cover ostomy/wound care services, but they may require prior authorization. The cost you are responsible for, such as a co-payment, co-insurance, or deductible, will be determined by your insurance company.

 

Our providers accept government-sponsored plans like Medicare and Medicaid, as well as many commercial plans like Blue Cross Blue Shield, United Healthcare, Aetna, Humana, among others.

 

However, exceptions may apply, so please check with our Wound Care Center or your insurance company for confirmation.

Do you offer patient education?

Yes.


We believe that knowledge is power, and that's why we offer a variety of patient education resources. From informational brochures to in-person patient education sessions, we are here to help you learn all you need to know about living with an ostomy.

If you have an existing ostomy, you still may have questions about how to manage your stoma and ostomy pouch. Our ostomy care specialists can provide guidance on skin care, pouch selection, and other aspects of ostomy management.

During your first appointment, you can expect the following:

  • Initial assessment: Questions will be asked about your medical history, symptoms, and any current concerns you have. The ostomy supplies you are currently using will be evaluated. This will help us gain a better understanding of your ostomy care needs.

​

  • Physical examination: Assessment of your stoma, parastomal skin and any wounds you may have.

​

  • Diagnostic testing: If necessary, diagnostic tests such as cultures, tissue samples, urine or stool samples, to help diagnose any underlying conditions.

​

  • Treatment plan: The treatment plan is tailored to you based on the assessment and diagnostic results.  This may include a combination of medication, lifestyle changes, and follow-up appointments.

​

  • Discussion of results and next steps: The results of the assessment, diagnostic tests, and treatment plan will be discussed with you and any questions you may have will be addressed. You will also discuss any follow-up appointments or referrals that may be necessary.
